AlohaCare Advantage Plus (HMO D-SNP)
To enroll into AlohaCare Advantage Plus:
- You must be a resident of Hawaii
- You have full benefit Medicaid coverage and are enrolled in AlohaCare's QUEST (Medicaid) health plan.
- You must have Medicare Parts A and B
- You must continue to pay for your Medicare Part B premium. The State pays the Part B premium for full-dual members who are eligible for AlohaCare Advantage Plus.

Important Enrollment Notice: AlohaCare Advantage Plan
Effective as of December 1, 2025, we are no longer accepting new applications for the AlohaCare Advantage (HMO D-SNP) plan.
This action is taken to align with our commitment to developing a fully integrated care program for our members in Hawaii, and in anticipation of upcoming guidelines and regulations from the Centers for Medicare & Medicaid Services (CMS) and the Hawai’i Med-Quest Division.
What This Means for You
- Current Applicants: If you submitted an application before 12:00 AM midnight HST on December 1, 2025, it will be processed according to the relevant plan and CMS rules.
- New Applicants: We will not accept or process AlohaCare Advantage plan enrollment forms, applications, or requests for enrollment received on or after this date. If you are a current or eligible QUEST member, we encourage you to explore our AlohaCare Advantage Plus (HMO D-SNP) plan.

- Other Enrollment Options: This restriction applies only to the AlohaCare Advantage plan. We can still assist you with information and enrollment for other available Medicare-related options.
