What is Community Care Services?

AlohaCare’s Community Care Services (CCS) is a managed behavioral health care plan. It's for Medicaid members who live with a serious mental health issue.

Who is Eligible? 

Members may be referred to the AlohaCare CCS plan for behavioral health services if they are:

  1. Covered under the QUEST Integration (QI) medical assistance program; and
  2. Diagnosed as having a serious mental illness
  3. Eighteen (18) years or older.

How Do I Apply?

  • See your qualified mental health professional:
    • Psychiatrist
    • Licensed Clinical Psychologist
    • Licensed Marriage and Family Therapist
    • Licensed Mental Health Counselor
    • Advance Practice Registered Nurse
    • Licensed Clinical Social Worker
  • Ask him/her to complete the 1157 Referral for Serious Mental Illness (SMI) Community Care Service (CCS) Program which also includes supportive documentation.
  • Current assessment, progress note or psychiatric/psychological assessment.
  • Your provider can login to our secure provider portal to access the application form.

  • Case management Services
