- We were established in 1994 by Hawai’i’s community health centers.
- We are proud to be a part of the State of Hawaii Department of Human Services CCS program
Your AlohaCare CCS team includes more than 282 employees on:
- Kaua'i, O'ahu, Moloka'i, Maui, Lana'i, & Hawai'i island
through your member portal or our Document Request form, or by calling AlohaCare CCS Member Services at 808-450-3386 or toll-free 1-888-397-9899. TTY/TDD users can call 1-877-447-5990. We will send it to you within 5 business days from the date we receive your request. This will be at no cost to you.
