AlohaCare has programs to help with certain medical conditions at no cost.
These programs help members care for their medical conditions, live healthier, and prevent time in the hospital or emergency room.
Members living with a chronic disease can receive support by a care team. Care team members offer service coordination, and useful information to manage health better and stay active.
AlohaCare’s disease management programs address the following medical conditions at zero-cost:
- Hypertension
- Diabetes
- Asthma
- Chronic Kidney Disease
