The AlohaCare Advantage and AlohaCare Advantage Plus formulary is a list of covered drugs selected by our plan in consultation with a team of health care providers, which represents the prescription therapies believed to be necessary part of a quality treatment program. AlohaCare Advantage and AlohaCare Advantage Plus covers both brand name drugs and generic drugs. Generic drugs have the same active-ingredient formula as a brand name drug. Generic drugs usually cost less than brand name drugs and are rated by the Food and Drug Administration (FDA) to be as safe and effective as brand name drugs. Generally, generic drugs cost lest than brand name drugs. AlohaCare Advantage and AlohaCare Advantage Plus may may change during the year. We may add or remove drugs from our formulary. If your drugs are not covered, you can fill out this Request for Medicare Prescription Drug Coverage Determination form to request an exception. By requesting an exception, you are asking us to cover your drug even though it is not on our formulary.
