2016 Health Outcomes Survey (HOS)

Each year Medicare reaches out to Medicare Advantage members through the Health Outcomes Survey (HOS).  This mandatory annual survey is sent to a sample of Medicare members and asks about members, their health and the quality of their health care.  The valuable data gathered through the Health Outcomes Survey helps AlohaCare improve our member experience, our provider network, and the quality of health care members receive. 
As an AlohaCare member, you may receive a letter in late March or early April from DSS Research, our certified vendor, notifying you of this survey.  If you are selected to receive a survey, it will be mailed to you in early April. You may receive a follow up phone call from DSS Research if a survey is not completed. We appreciate your cooperation by completing the survey and mailing it back by the date indicated on the survey.
Please be assured that your confidentiality will be maintained throughout this entire process.  Member feedback that you provide through the survey remains anonymous and AlohaCare will only receive overall results that do not identify any member information.

AlohaCare is a local, non-profit health plan. We were founded in 1994 by Hawaii’s Community Health Centers. AlohaCare is the third-largest health plan in Hawaii.

We specialize in QUEST Integration health insurance to people living on Oahu, Kauai, Molokai, Lanai, Maui and the Hawaii Island. We are mission driven to care for people who are underserved with specific health needs.

We provide health care coverage for Hawaii's QUEST Integration (Medicaid) beneficiaries. AlohaCare is a health plan with a Medicare contract and provides Medicare coverage to beneficiaries in Hawaii.
