Enrollment into Our Health Plans


Becoming an AlohaCare member is easy.
 
We’ve made the enrollment process smooth for you by putting together a quick guide to enrolling in our plans. You’ll find the enrollment basics of what you need to know about each plan below. Be sure to review all the information before making a decision, because there are several things to consider. For example, there are eligibility requirements you must meet, and in some cases, there are certain time frames you can enroll.

alohaCare


You must apply with the State for QUEST eligibility. Once you become QUEST-eligible, you will have 10 days to choose a health plan. At this time, you will be able to choose AlohaCare as your health plan or for your family members. We serve QUEST members statewide.
 
Are you currently a QUEST member? Would like to change your health plan to AlohaCare? You can make the change during the QUEST open enrollment. It is from May 1 though May 15. Except for this annual open enrollment and some exceptions, you must remain with your health plan for the entire year.

ALOHACARE ADVANTAGE (HMO)


To enroll into AlohaCare Advantage, you must live in Hawaii and have Medicare Parts A and B. You must continue to pay your Part B premium if not otherwise paid for under Medicaid or by another third-party.  
 
You can enroll into AlohaCare Advantage from November 15 to December 31 of every year, during the Annual Election Period (AEP). You will become effective on our plan beginning January 1. 
 
From January 1 through March 31 you may switch plans once, but cannot add or drop Prescription Drug Coverage. Your coverage will be effective the first day of the month after we receive your completed application.

The AlohaCare Medicare Prescription Drug Benefit is only available to members of AlohaCare. If you are already a beneficiary enrolled in a Medicare Advantage Prescription Drug plan, you must receive your Medicare Prescription Drug Benefit through that plan. If you obtain routine care from out-of-network plan providers neither Medicare nor AlohaCare will be responsible for the costs. Our contract with CMS is renewed annually and the availability of coverage beyond the current year is not guaranteed.

  2. Enroll by phone: Speak to an Enrollment Specialist who will help you enroll over the phone. Call 973-6490 or toll free at 1-877-812-0249 from Oahu, Neighbor Islands or Mainland. We are available 8 a.m. to 5 p.m., Monday through Friday. 
     
  3. Enroll by mail or fax: Click on the Enrollment Form.

  4. Enroll through Medicare: Medicare beneficiaries may enroll into AlohaCare through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.medicare.gov. For more information contact the AlohaCare from Oahu at 973-6395 or toll-free at 1-866-973-6395 from Oahu, Neighbor Islands or Mainland, 8 a.m. to 8 p.m., Monday through Friday, or visit us 8 a.m. to 5 p.m., Monday through Friday. ALOHACARE ADVANTAGE PLUS (HMO SNP)


To enroll into AlohaCare Advantage Plus, you must live in Hawaii, have Medicare Parts A and B and have Medicaid. You must continue to pay your Part B premium if not otherwise paid for under Medicaid or by another third-party.
