Enrollment into Our Health Plans
Becoming an AlohaCare member is easy. You’ll find the enrollment basics of what you need to know about each plan below. If you have any questions about the enrollment process, don’t hesitate to contact us with your questions. Our representatives are standing by and are happy to help.
alohaCare QUEST
You must apply with the Med-QUEST Division for QUEST coverage. Once you become QUEST-eligible, you will be able to choose a health plan. At this time, you will be able to choose AlohaCare as your health plan or for your family members.
Choose AlohaCare as Your Health Plan
You will receive a QUEST Enrollment Packet. Choose AlohaCare as your QUEST health plan and return the packet. If you do not choose a health plan, you will be automatically enrolled into a health plan.
Would you like to change your QUEST health plan to AlohaCare? You can make the change during the QUEST Open Enrollment Period.
ALOHACARE ADVANTAGE (HMO)
To enroll into AlohaCare Advantage:
- You must be a resident of Hawaii
- You must have Medicare Parts A and B
- You must continue to pay your Part B premium
You may enroll in the plan only during specific times of the year. Contact AlohaCare for more information.
- Enroll online: This simple process can be completed on your own time and in the convenience of your own home. To begin to online enrollment process, click on the red Enroll Now! button in the upper left part of the webpage.
- Enroll by phone: Speak to an Enrollment Representative who is ready to assist you. Call 973-6490 or toll-free at 1-877-812-0249, 8 a.m. to 5 p.m., Monday through Friday.
- Enroll by mail or fax: Fill out the 2013 Enrollment Form and mail or fax it to us at:

- Enroll by phone or online with Medicare: You may enroll over the phone by calling 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048. Medicare is available to help you 24 hours a day/7 days a week.
Medicare beneficiaries may also enroll into AlohaCare through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.medicare.gov. (By clicking on this link, you’ll be leaving the AlohaCare website.)

ALOHACARE ADVANTAGE PLUS (HMO SNP)
To enroll into AlohaCare Advantage Plus:
- You must be a resident of Hawaii
- You must have Medicare Parts A and B
- You must continue to pay your Part B premium. The State pays the Part B premium for individuals with full benefit Medicaid coverage, who eligible for AlohaCare Advantage Plus.
- You must have full benefit Medicaid coverage
You may enroll in the plan at any time during the year. Contact AlohaCare for more information.
