To enroll into AlohaCare Advantage:

• You have Medicare Parts A and B
• You live in the State of Hawaii
• You must continue to pay for your Medicare Part B premium if not otherwise paid for under Medicaid or another third-party
• You do not have End Stage Renal Disease (ESRD) at the time of enrollment

To enroll into AlohaCare Advantage Plus:

• You have Medicare Parts A and B
• You live in the State of Hawaii
• You must continue to pay for your Medicare Part B premium if not otherwise paid for under Medicaid or another third-party.
• You have full-dual Medicaid coverage
• You do not have End Stage Renal Disease (ESRD) at the time of enrollment
